Why do you need a medical examination
Clinical examination, or as it is also called screening, in a general sense, is a mass examination (in the health care system) of the population of Russia, which implies a set of measures carried out to prevent and further identify various diseases in the population, to prevent their development.
The following process is a clinical examination - medical (preventive) examinations of the population, medical research and consultations of doctors are carried out, which are carried out in a person in his predetermined age cycles (periods).
The main goals of clinical examination in 2022:
1. Earlier identification of chronic non-communicable diseases, which are the main causes of disability and premature mortality of the population of the Russian Federation, which include:
- diseases of the circulatory system and, first of all, ischemic heart disease and cerebrovascular diseases;
- malignant neoplasms;
- diabetes;
- chronic lung disease.
These diseases are responsible for more than 75% of the total mortality of the population of our country.
2. Identification and correction of the main risk factors for the development of these diseases, which include:
- high blood pressure;
- high blood cholesterol levels;
- elevated blood glucose levels;
- smoking tobacco;
- harmful alcohol consumption;
- poor nutrition;
- low physical activity;
- overweight or obesity.
To whom and when to pass
Law No. 323-FZ "On the Basics of Health Protection of Citizens of the Russian Federation" states that all citizens of the country who have reached the age of 18 are subject to compulsory medical screening, regardless of the form of employment and place of work.
Clinical examination has been introduced in Russia since 2013 as part of the compulsory health insurance program. Patients aged 18 years and older undergo an annual medical examination or preventive medical examination.
You can undergo medical examination:
- 1 time in three years at the age from 18 to 39 years inclusive (at 18, 21, 24, 27, 30, 33, 36, 39 years old);
- annually aged 40 and over.
According to the provisions of the Law on the Protection of the Health of Russian Citizens, the employer “… is obliged to provide conditions for employees to undergo medical examinations and medical examinations, as well as to release employees to undergo them without hindrance …” (Article 24 of the Federal Law No. 323).
If the employer requires confirmation that the employee was undergoing medical examination, then a certificate may be issued at the place where it was carried out, indicating the time spent on its passage.

What is included in the clinical examination
In 2022, the clinical examination will traditionally take place in two stages.
1st stage:
- preventive medical examination (see above);
- a general blood test, including the determination of the level of hemoglobin, leukocytes and ESR;
- a set of special tests for cancer (see above).
2nd stage:
- examination by a neurologist according to medical indications. In particular, if there is a suspicion of a previous acute cerebrovascular accident;
- duplex scanning of brachycephalic arteries.This is an expensive study of the main arteries that supply blood to our brains. Such an examination is prescribed for men 45 - 72 years old and women 54 - 72 years old if they have three risk factors at the same time: high blood pressure (140/90 mm Hg and above), cholesterol (above 5 mmol / l) and overweight ( it is determined by the doctor during examination by measuring height, weight and waist circumference). And also this examination is assigned to persons 65 - 90 years old in the direction of a neurologist if a previous stroke is suspected;
- examination by a surgeon or urologist. For men aged 45, 50, 55, 60 and 64 years old with an increase in the level of prostate-specific antigen in the blood of more than 4 ng / ml;
- examination by a coloproctologist, including sigmoidoscopy - according to medical indications and directions of other specialist doctors;
- colonoscopy. As prescribed by a surgeon or coloproctologist if you suspect colon cancer;
- esophagogastroduodenoscopy. As prescribed by a general practitioner with suspected cancer of the esophagus, stomach and duodenum;
- radiography of the lungs, as well as computed tomography of the lungs. As prescribed by a general practitioner if lung cancer is suspected;
- spirometry. By appointment of a therapist for smoking citizens and with suspicion of chronic bronchopulmonary disease;
- examination by a gynecologist. For women with adverse changes found during examinations for cervical cancer, as well as for breast cancer;
- examination by an otorhinolaryngologist. According to medical indications for citizens 65 years of age and older;
- examination by an ophthalmologist. For citizens 40 years and older who have increased intraocular pressure, as well as 65 years and older with reduced visual acuity that cannot be corrected with spectacles.

- using the Moscow State Services mobile application;
- using information kiosks located in a medical organization;

Is it possible to refuse medical examination
Clinical examination is carried out with the informed voluntary consent of the citizen. A citizen has the right to refuse to conduct clinical examination in general or from certain types of medical interventions included in the volume of medical examination, while he must give a written refusal in the form and in the manner approved by Federal legislation.
Immediately before the prophylactic medical examination, you must sign a voluntary informed consent form. In case of refusal from medical examination - a form of official refusal.
You can also refuse to part of the research that is carried out as part of the clinical examination. However, if their volume exceeds 15 percent, the clinical examination will not be considered passed.
The employer is interested in that his employees regularly undergo medical examinations, as this will reduce the incidence and the number of working days spent on sick leave. Therefore, he can motivate employees to undergo medical examination. However, it is illegal to force employees to do this.
Taking care of your own health is the duty of every citizen. The state provides citizens with the opportunity to obtain comprehensive information about the state of health, prevent the development of chronic diseases by reducing increased risks and begin treatment of existing diseases at the earliest stage. However, it is everyone's personal choice to undergo or not undergo medical examination.No sanctions can be applied to those who refuse medical examination.
